“Burke’s Law” and How Dr. Dale Bredesen Just Elevated It to the Spotlight
Last weekend at the Personalized Lifestyle Medicine Institute (PLMI) Conference, the energy was electric. It felt like the epicenter of the new paradigm in brain health, an extraordinary gathering of the leading minds working to reverse cognitive decline. On stage were icons like Dr. Robert Lustig, who unpacked the role of reactive oxygen species in neurodegeneration, and Drs. David and Austin Perlmutter, authors of Grain Brain and Brain Wash, exploring the immunological and metabolic drivers of brain dysfunction.
And then there was Dr. Dale Bredesen, delivering his signature closing keynote on the science and practice of Alzheimer’s reversal. But it was at the very end of his talk that he dropped something new, something that rippled through the audience audibly.
He has decided to call it ‘Burke’s Law’.
What Is Burke’s Law?
Dr. Kristine Burke, one of the leading clinicians in precision brain health and TruNeura’s co-founder. It’s the distilled wisdom from years of real-world experience successfully helping patients not just slow, but reverse, cognitive decline.
At its core, Burke’s Law states:
“Lifestyle modification is necessary but not sufficient for reversing cognitive decline.
Without addressing underlying causative factors, the improvements from lifestyle alone will fade within 6–12 months.”
It’s a simple truth that captures why so many “miracle” lifestyle interventions fail to produce lasting results. You can eat clean, meditate daily, and get perfect sleep—but if chronic infections, toxins, vascular dysfunction, or immune dysregulation remain unresolved, your progress will plateau and regress.
Why Lifestyle Alone Isn’t Enough
Consider Dr. Dean Ornish’s well-publicized study on lifestyle interventions for cognitive decline. The results were inspiring - proof that nutrition, exercise, stress management, and social connection can all improve brain function.
But the study lasted just 20 weeks. It didn’t (and couldn’t) capture the sustainability question: What happens a year later?
Burke’s Law predicts the answer—without addressing the root causes, the early gains fade. Patients often feel better at first, but the deeper biological stressors continue to wear on the brain, ultimately undoing the benefits of even the most disciplined lifestyle practice.
The Precision Medicine Difference
The only documented long-term reversals of cognitive decline come from precision medicine approaches, like those pioneered by Dr. Bredesen and now practiced daily by clinicians like Dr. Burke.
This model builds upon the foundation of lifestyle medicine but goes deeper—identifying and treating the specific biological imbalances that drive neurodegeneration. In TruNeura, we think about these as the six root-cause subsets—the major categories of dysfunction that, if left unaddressed, continue to degrade brain performance even in the face of healthy habits.
TruNeura: Built on Burke’s Law
Our platform helps clinics operationalize this two-phase process of cognitive restoration:
The Eight Pillars of Health — Move, Rest, Nourish, Connect, Reflect, Grow, Compose, and Remove.
These are all verbs, because they represent the actions patients must take to build a healthy foundation. TruNeura’s software is built to coach, remind, and reinforce these behaviors consistently over time. The Six Subsets of Root Cause — the clinical domains practitioners must address to remove the deeper drivers of decline. TruNeura empowers practitioners to track and manage these complex, interrelated systems efficiently within a single workflow.
In short, TruNeura exists to make the necessary and the sufficient possible—helping practitioners deliver both the behavioral transformation and the root-cause resolution that true reversal requires.
From Law to Practice
Burke’s Law is more than a theory—it’s a clinical reality. It’s the reason why some patients get their lives back while others continue to slip away despite doing “everything right.”
And it’s why the future of brain health belongs to those who combine the art of lifestyle medicine with the science of precision diagnostics and treatment.

Let’s make reversal the new standard of care.

Amazing work, Kristine! Love what you do and wonder if you have any advice on how to get changes in the STANDARD OF CARE and ETHICS STANDARDS to state that root causes of brain disorders must have adequate tests and treatments that have been PROVEN to work. Did you know that dementia, mental illnesses, autism, Parkinson's disease, chronic joint diseases have NO requirement to be tested for most root causes. Lyme, Bartonella, heavy metals, mold, mycotoxins, etc should be required!!! Breitschwerdt showed Bartonella in 11 of 17 schizophrenic pts and in 43% of psychotic patients and tells of a 14 yr old boy w 4 psych hospitalizations before anyone knew that skin stretch marks are a tipoff of Bartonella. Yet no one is testing for it and when I presented it to iECHO psychologists, they were not very interested in it or in learning from the NEURO iECHO consortiums, etc. APA, AAP are way behind the science too...finally admitted that PANDAS might be real. Lyme denied for 45 years. . So it will take a STANDARD OF CARE requirement to get it done. Let's try to get advocates of all organizations to help get it changed. I post a lot on Linked In, hosted a BRAIN DAY EVENTS at NEOMED with experts who were amazing, volunteered, included AlzPi.org, IntellxxDNA. org, Dr SHemesh who found HSV triggers tau, Mind-Eye Institute who fixes with eyeglasses w prisms, featured in the book "The Ghost in my Brain.... Yet almost ZERO interest at NEOMED. They do not teach any of this so another generation is going to be misdiagnosed. Any input is welcome. Your fan, Teri Sanor, MD
